Our clinical case managers work with doctors, Hospice Care providers, and hospital discharge planners to ensure children transition effectively back home to maximize their stabilization and recovery and minimize the chances of rehospitalization. Our long-term skilled nurses (Private Duty Nursing) care for chronically fragile children and are trained to manage the technologically advanced equipment they may need to manage respiratory and feeding needs and remain safely stable at home.

Our highly screened and licensed Registered Nurses (RN) and or licensed Practical Nurse (LPN) deliver quality care and exceptional service while working closely with the family to develop an environment which nourishes the physical, mental, and emotional development of each child. As leading experts in pediatric home care, we understand the daily challenges of caring for a medically fragile child and the importance of providing support for the family.

Whether it is 24 hours a day, 7 days a week or a few 12-hour shifts per week, our nursing services are available 365 days and nights per year to meet your specific needs. We are committed to working as part of the team of medical professionals involved with your child’s care to coordinate and implement the best possible comprehensive plan of care.
